The Main Menu
The Main menu for the Forms module is below. There are many fields
and buttons that need to be considered.
Print List- Click this if you want
to first review and than print a list of all records currently in the
forms module
Delete- Clicking this deletes
all record in the module after you confirm your action at the dialog
below.
- Recall
Method- This field presets the recall method you want to use on
the current computer. The methods are Costar, Labels, Letters,
Postcards.
- Sort by- This determines how
the current records are sorted before printing. The selections are Chart, Patients, Zip codes.
- Click
to edit a standard layout- These buttons take you to the
specific layout so you can either edit it. There is more information
about editing in another
section.

Custom Form Layouts- Besides the standard layouts above, there
are 12 others for you to customize. Except for the 1st (Superbill),
11th (Thank you note)
and 12th (New Patient Information), you can edit the names of them.
These names appears in a
selection dialog when you want to print a "Custom Form" from the Patients Module.
To edit one of the layout names, first
select the one to edit from the pop-up list (B). Next, click the Edit
Label button (C) to get
the first dialog below where you edit that layout's name. This is the
name that appears in the sample selection dialog above! However, if you
try to edit one of the restricted names, you will get the second dialog
below!
Default
Layout Selection
You can select the default custom form that will always be
printed for a patient. You make this selection in the Default Custom Form field. In Ask user for custom? you get to
select if the user is ever asked to select a custom formor can actually
select from the selection dilog shown above.
Physician
Automated Messages
If you do not have PAM loaded or are running a
Macintosh, you can only print a list of patient appointments. Thi
system takes the appointments for a day, dials the phone and in a human
voice tells them they have an appointment. This is an add-on feature
and requires a dedicated PC.
